Application of membership

Applicants for memberships should bring the following documents:

1. Certificate of diploma or degree

2. Photocopy of Singapore registered TCM Physician Certificate

3. Certificate of Masters’/Doctorate degree

4. Recent 2.5passport size photograph

5. Cross cheque of S$100 made payable to the"Society of Traditional Chinese Medicine(Singapore)"

6. Complete the attached application form for member
